Home Health care is a great option for seniors and those who need medical care but have difficulty getting out of their homes to seek/receive the help they need. Home health care provides a wide range of services in the comfort of your own home. The goal being to assist you in your recovery through education and treatment to maximize independence.
Have you been diagnosed with a new disease? Are you managing a long-term illness, or recovering from an accident, illness or surgery? If so, you may qualify for Home Health care as part of your Medicare benefit? Medicare participants are eligible for home health care benefits as long as they meet the Medicare requirements for homebound status and the medical need for skilled services.
Available Home Health Services
Skilled Nurses provide individualized care and case management in multiple areas including diagnosis specific education in coordination with your physician, intravenous therapy, wound care, pain management, medication management, and nutritional teaching.
Our licensed Physical Therapists are experts in helping patients decrease risk from falls; recover from joint replacement and other orthopedic surgeries; adapt to stroke related disabilities, Parkinson’s disease and Multiple Sclerosis; and maximize function associated with cardiac and pulmonary disorders. Physical Therapists assist individuals in improving their ability to perform the functional tasks of mobility, walking, balance, energy conservation and day to day activities.
Occupational Therapists conduct comprehensive evaluations of a client’s home, recommend necessary adaptive equipment, and assist patients in personal care such as bathing, grooming, dressing, and toileting. Our Occupational Therapists treat with a holistic approach focusing on adapting the environment to fit the patient and addressing safety needs.
Speech Therapists evaluate and treat communication/speaking skills, eating/swallowing deficits and cognitive difficulties. They work with family members and caregivers to provide strategies that facilitate and maintain functional communication. Speech Therapists help improve swallowing skills and teach compensatory strategies that help reduce the risk of aspiration pneumonia, maintain adequate nutrition and hydration.
Our Home Health Aides work in coordination with the patient, clinician, and physician to foster independence and safety in the areas of personal care. Home Health aides can assist with activities of daily living such as bathing, dressing, toileting, and light meal prep/housekeeping.
Social Workers consult with both the patient and their family regarding financial needs, emotional support, adjustment to illness, and provide the link between patients, their families and community resources.

Getting Started
First, we begin care by performing an initial health assessment. Next, we collaborate with patients, physicians, and any caregivers to develop a treatment plan. We then provide the appropriate skilled health care professionals to help patients reach their home health care goals.
Qualifications
To receive services Medicare requires beneficiaries to be homebound, which is defined as requiring assistance to leave home and leaving home is difficult and taxing. Patients must be under the care of a physician and the services we provide must include intermittent skilled nursing or rehabilitative therapy.
Cost
Traditional Medicare covers our services at 100%. We are also in network with most Medicare replacement plans and private insurance companies. For Medicare replacement and private insurance, benefits differ so verification of benefits is required prior to initiating services.
